GuidelinesIMMEDIATELY REPORT
It is imperative that you
immediately report all losses within 24 hours of the loss. Report losses to the
24 hour toll free CLAIMCARE number: ( 888) 80 CLAIM (25246).
Online Reporting
You can now submit your
Commercial Auto and Inland Marine claims on the
internet. If you are reporting a General Liability
or Property claim, complete the General Liability
or Property loss notice and fax it to CLAIMCARE -
(800) 809 0642.
Please provide details of your claim on the web-enabled loss
notice. Provide the facts of the loss, all persons involved (including phone
numbers) and injuries. For Commercial Auto claims, enter all the vehicles
involved and any property damage that occurred during your loss. In order to
expedite the online process, please have the appropriate loss and policy
information before you begin.
Enter all the information that you know
about your policy and the details of the loss (e.g. policy number, insured
information, contact information, loss information, property damaged, injured,
witnesses, etc…). For Commercial Auto claims, if more than five vehicles
or persons are directly involved in the loss, please contact CLAIMCARE at (888)
802-5246.
Fax Reporting If you can not report the loss by
phone or complete the online loss notice, fax the loss notice to the CLAIMCARE
fax number - (800) 809 0642 or mail it to the following address: CLAIMCARE -
6363 N. State Highway 161, Suite 100 - Irving, Texas 75038.
