To Initiate the Contracting Process
The first
step in the contracting process is to submit the Prospective Agent Information
Sheet to Deep South. Download the form, then fax the completed form to
the fax number shown on the Office Locations
page. If, based on our review of this information, we determine that we
should proceed further with contracting, we will provide an application to be
completed and returned to your Office
Location.
Completion of the contracting process will then be
contingent upon our receipt and review of the following items:
- Completed Application
- Completed Bank
Information
- Copy of E & O Dec
Page
- Copy of All Licenses
- Physical Visit to
Agency
- Review and Approval by Management
